The Glasgow Coma Scale records eye opening, verbal response and best motor response as three separate observations, scored out of 4, 5 and 6. Any component can be marked not testable, and then no total is produced rather than a guess. The assessment itself is made at the bedside by a clinician trained in the scale, and a score is one observation over time that determines nothing on its own.
